The Supporting Documents You Will Need

If you are making an Emergency Medical Insurance claim, we may need:

  • Original itemized receipts for all bills and invoices.
  • Proof of payment by you and by any other benefit plan.
  • Medical records including complete diagnosis by the attending physician or documentation by the hospital.
  • Proof of the accident if you are submitting a claim for dental expenses resulting from an accident.
  • Proof of travel (including departure date and return date).
  • Your historical medical records.

If you are making a claim for expenses related to a travel delay, we may need:

  • Original unused transportation tickets and vouchers.
  • Original passenger receipts for the new tickets you had to purchase.
  • Original receipts for the travel arrangements you had paid in advance and for the extra hotel, meal, telephone and taxi expenses you may have had.
  • Any other invoice or receipt supporting your claim.

If you are making a claim for expenses related to delay of baggage or travel documents we may need:

  • A Police report or an incident report from the hotel manager, tour guide or transportation authorities.
  • Copies of reports from the authorities as proof of loss, damage or delay.
  • Proof that you owned the articles, and receipts for their replacement.
  • Flight/travel itineraries
  • A copy of your flight/travel itineraries indicating your flight details
  • Medical report or discharge summary or doctor’s memo
  • Written confirmation from the hospital stating date, cause and nature of injury/illness
  • Original medical bills and other expense receipts
